Dear Faculty Advisor: Each year the Internal Revenue Service requires that a chapter of Delta Phi Alpha which has been declared tax exempt under a group exemption letter file with the National Office a letter of authorization for two purposes: 1) to include the chapter in the group return for the upcoming tax year, and 2) to comply with the revised Group Exemption Procedures. What this means is that you are respectfully requested to complete the “Letter of Authorization” and return it to me as soon as possible. If your chapter is not included in the annual group report, which goes out in the spring, you may be subject to an IRS audit. Please note that the form asks for an estimate of income, not an actual documented amount. This year for the first time, we are trying to get the authorization letters sent to the national office electronically. You can download the form and mail it to me or fax it to 434-395-2145.
The Employee Identification Number (EIN) was issued soon after your chapter was founded. (This is not your Social Security Number.) It should look similar to this: ## #######. The first two #s are often 23 or 94 or 52. Even if you do not have this number, send in this form and we will send you an application for this required number. If you do not know your number for any reason, please complete the form anyway. New chapters must submit an SS-4 form to me to obtain an EIN. That form is filed with the annual group report. The IRS then issues the EIN to the chapter advisor.
